STEP 1: Complete and Submit an Intake Survey
Once a profile has been established, the next step is to complete an intake survey to create a case. Intake surveys AKA Initial assessments are used to capture initial data collection fields for a potentially eligible client in a project. Common intake questions include but are not limited to:
- Program consent
- Specific PII/ PHI demographic info
- Location details
- Household details
- Program-specific data gathering
All surveys hosted in Caresuite are uniquely built by your organization's admin team. If you are experiencing issues completing a survey, please reach out to your system administrator.
Tips when completing Surveys
Mandated Fields
Mandated fields always show a red asterisk (*) in front of them. Be sure to answer these in order to successfully submit a survey
Collapsed Question Groups
If you see a header with a sideways triangle next to it, this is a collapsed group of questions. Click the triangle to expand the questions.
Selecting from large picklists
Sometimes select questions have a lot of options to choose from,begin typing the option you are looking for to hop to the correct select option.
Some fields have formatting requirements/ restrictions. To complete these field types be sure to read the instructions in the question hint and in the error message.
NEXT = Submit
For surveys that are not multiple pages, the NEXT button is always the submit button for a survey.
For surveys that have multiple pages, it serves as both a next page and a submit button.
